Our Story

Angels Grace Hospice & Palliative Care began with people who share a passion for hospice.

We believe that exceptional end-of-life care can only happen when highly trained, experienced professionals are dedicated to listening to patients and their families, focused on respecting their ideas, and committed to making a difference.

Our leadership team includes medical social workers and nurses with extensive hospice experience. In fact, over 50% of our nurses are Certified Hospice and Palliative Nurses (CHPN) and our medical director is Certified in Hospice Palliative Medicine.

All of our employees are hand-picked to fulfill our vision.

We deliver state-of-the-art hospice care services with identified centers of excellence. We engage in a wide range of continuing education for hospice personnel and volunteers, as well as other educational programs for professionals and the public.

Our Policy

Goals

  1. To provide a program of care that acknowledges death as a normal event.
  2. To provide direct patient care in the home, and family/caregiver support in a coordinated manner with an interdisciplinary program using existing resources and avoiding duplication of services.
  3. To offer short-term inpatient care when the patient’s pain and symptoms must be closely monitored in order to be controlled, or when the family/caregiver needs a rest from the tedium and stress involved in caring for the patient.
  4. To maintain a commitment to realistic, cost-effective management, utilizing all available mechanisms for payment, to retain management responsibility for the patient’s care in the utilization of direct and indirect services.
  5. To make hospice and the hospice philosophy an accepted part of the health care system and to increase awareness concerning the needs of the dying.
  6. To provide a continuum of care that recognizes and responds to the physical, social, spiritual, emotional, and other needs of the patient and family/caregiver.
